A name nobody put back
Two days earlier, a rebuild script stripped the name off the image the main door was running. Nothing broke right away, because a running container does not care what its image is called. A review even wrote the problem down. Nobody fixed it. Then the host rebooted without warning, and on the way back up the service went looking for its image by name, found nothing, and retried hundreds of times without getting anywhere. The lesson is not really about images. A review that spots something dangerous in the live system has to open a job for it before the review counts as done. A small check now rejects a review note that names a live hazard without pointing at a job.
Silent is failed
The fix script ran, and the person running it saw nothing. Its output went to a log file and its failure went to a status file nobody was watching. From the chair, a script that says nothing looks the same as one that is hung, working, or already dead. New rule for every script a human runs: a line as each step starts, a heartbeat during long waits, and a last line that says done or failed. Any command the owner has to paste comes in its own copy block, so one tap copies it and nothing needs hand-selecting.
Map before you touch
The outage was two old changes meeting a reboot. Nobody could have seen that by looking at one change at a time. So before anything else got fixed, the agents wrote a map of the host: what each piece depends on, what breaks it, and how to bring it back, with anything they could not see marked as unseen. Uptime alerts to a phone are drafted but not installed. They wait on the owner's go, because installs are the kind of thing that is costly to undo.
Write it down when it is said
An idea got lost when the chat it lived in was deleted. It was pieced back together and saved to a file. The rule now: when a new idea, plan, or decision comes up, it goes into a file the same turn, and every chat gets closed out to files before it is deleted. Chat is air.
Say where the number came from
An assistant reported a count of unpushed changes as fact. The number came from a stale snapshot and it was wrong. The push script now fetches fresh before every run and labels its comparison fresh or stale, with a timestamp. A number without a source is a guess.
